Who qualifies for semaglutide is usually the very first question people ask when they reach out to us, and it's a smart place to start. Eligibility for a GLP-1 weight-loss medication isn't decided by a quiz or a number on the scale alone. It comes down to your health history, your current medications, your goals, and a clinical evaluation with a licensed provider.

I'm Amanda Walrath, APRN, FNP-BC, a Florida-licensed, board-certified family nurse practitioner with 17 years of clinical experience and the founder of Nuvo Wellness and Health. I started Nuvo after my own weight-loss struggle after having kids, and I've taken the same medication I offer. In this guide I'll walk you through the general guidelines providers use, the health factors that matter, who should be cautious, and what the screening process looks like when you start care with us by telehealth anywhere in Florida.

Compounded semaglutide and tirzepatide are not FDA-approved, and they are not the same as, equivalent to, or generic versions of any brand-name medication. Whether treatment is appropriate for you is decided by a clinical evaluation. Individual results vary.

Who qualifies for semaglutide? The general BMI guidelines

FDA-approved GLP-1 medications for chronic weight management are labeled for adults who meet one of these body mass index (BMI) guidelines, and providers commonly use the same framework as a starting point when considering GLP-1 treatment for weight loss:

Guideline
BMI
Additional requirement
Obesity range
30 or higher
None beyond a clinical evaluation
Overweight range
27 to 29.9
At least one weight-related health condition
Below 27
Under 27
Generally not a candidate for GLP-1 weight-loss treatment

BMI is an estimate based on your height and weight. It isn't perfect, since it doesn't measure muscle, body fat distribution or overall fitness, but it gives your provider a consistent place to begin. You can find a free BMI calculator from the CDC or NIH online if you'd like to check yours before your visit.

Weight-related conditions that factor into GLP-1 eligibility

If your BMI falls between 27 and 29.9, your provider will look for at least one weight-related condition. Common examples include:

–High blood pressure
–Prediabetes or type 2 diabetes
–High cholesterol or triglycerides
–Obstructive sleep apnea
–Heart disease

You don't need a formal diagnosis in hand before booking. Part of the evaluation is reviewing your history together, and in some cases your provider may recommend lab work or ask about recent results from your primary care provider.

Who should not take semaglutide or should be especially cautious

Meeting the BMI guideline is only half the picture. Safety screening matters just as much, and these are some of the factors I always discuss with patients before prescribing semaglutide or tirzepatide:

–A personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2). GLP-1 medications are not used in these situations.
–Pregnancy, breastfeeding, or planning a pregnancy. These medications are not recommended during pregnancy, and your provider will talk with you about timing if you're planning one.
–A history of pancreatitis.
–Gallbladder disease or a history of gallstones.
–Kidney problems, especially if you're prone to dehydration.
–Diabetic retinopathy or other diabetic eye disease.
–Severe stomach or digestive conditions, such as gastroparesis.
–Type 1 diabetes, or taking insulin or a sulfonylurea, which can raise the risk of low blood sugar when combined with a GLP-1.
–A history of depression or thoughts of self-harm, which your provider should know about so they can monitor you.
–A past allergic reaction to semaglutide, tirzepatide or a similar medication.

Having one of these doesn't always mean you can't be treated, but it does mean the decision needs a careful, honest conversation. If you take birth control pills, mention that too, because tirzepatide in particular may make oral contraceptives less effective for a period of time after starting or increasing a dose.

What the semaglutide eligibility screening looks like at Nuvo

Because Nuvo is a telehealth-only practice, your entire evaluation happens by video visit from home. Here's what I review with every patient:

1.Your height, weight and BMI, plus your weight history and what you've tried before.
2.Your medical history, including the conditions listed above.
3.Every medication and supplement you take, so we can check for interactions.
4.Your family history, especially thyroid cancer.
5.Your goals, schedule and comfort level with a weekly self-injection.
6.Whether semaglutide or tirzepatide is a better fit, or whether neither is appropriate right now.

If GLP-1 treatment isn't the right choice for you, I'll tell you honestly and explain why. Sometimes the best next step is to talk with your primary care provider, address another health issue first, or focus on nutrition, movement and sleep.

Qualifying for semaglutide vs. tirzepatide

The general eligibility guidelines for semaglutide and tirzepatide are very similar, and the same safety screening applies to both. The choice between them depends on your health history, how you've responded to medications in the past, side effects, and budget. At Nuvo, compounded semaglutide 2.5 mg/mL is $400 per 4 mL (10 mg) vial, and compounded tirzepatide 15 mg/mL is $500 per 4 mL (60 mg) vial. Each vial lasts about 12 weeks depending on your dose, and medication, supplies, shipping and ongoing follow-up are included, with no monthly membership.

If you do start treatment, Nuvo's GLP-1 Patient Guide covers storage, injections and when to call, and our semaglutide dosage chart post explains how doses translate into syringe units.
