The unit amounts below apply only to a 2.5 mg/mL concentration measured with a U-100 insulin syringe, where each unit is 0.01 mL (100 units equals 1 mL). The chart follows a typical schedule that raises the dose every 4 weeks.
Units to milligrams: what your syringe line means
If your label lists your dose in units, this chart shows the matching amount in milligrams for Nuvo's 2.5 mg/mL concentration on a U-100 syringe. Each unit is 0.01 mL, which is 0.025 mg at this concentration.
When I started semaglutide, I began on a very low dose and moved up slowly. That's the approach I recommend: start low, go slow, and let your provider guide every change.
How to convert milligrams to syringe units
Two numbers are needed: your prescribed dose in milligrams (mg) and the concentration printed on your vial label in milligrams per milliliter (mg/mL). Divide the dose by the concentration to get milliliters, then multiply by 100 to get units on a U-100 syringe.
This is why charts from other sources often disagree. At a 5 mg/mL concentration, for example, the same 0.25 mg dose would be only 5 units. A chart is only correct for the exact concentration it was written for, and the FDA has reported dosing errors when patients measured compounded semaglutide in the wrong units or volume.
Why "units" can be confusing
Milligrams measure the amount of medication. Milliliters measure liquid volume. Syringe units are markings that help measure that volume. The same number of milligrams can equal a different number of syringe units when the medication concentration changes. Never use a chart from another clinic, pharmacy, vial, or social media post unless the concentration exactly matches your prescription label and your clinician confirms the instructions.

Do not estimate a dose or copy someone else's instructions. If you are uncertain about the correct syringe line, pause and ask for help before injecting.
How semaglutide works
